To protect both you and our company from unauthorized credit card usage we require the following forms be completed and faxed back along with a copy of the front and back of the credit card.
Please submit the form within 30 minutes
after ordering. We can not guarantee your tickets will be held until your
form is received. No order is guaranteed until it has been confirmed.
If you do not have a fax or copier please visit a UPS Store, Staples, Office
Depot, etc. If you are a guest of a hotel ask the Front Desk for assistance.
Contact our office if you will take longer than 30 minutes to fax the form.

| This form is required for orders over $1,000, or if shipped to an address other than your billing address. Your order will be shipped to your billing address if this form is not received. |

| This form is required for orders that you would like dropped off without obtaining the required courier signature. Please note we take no responsibility if your tickets are left unattended and are lost or stolen. If you request a signature waiver please fax this form within 30 minutes of ordering. Your order will be shipped to your billing address and require a signature if the form is not received. The courier will leave a note on the door to make other arrangements if you are not available to sign for the tickets. |

| This form is required for orders
that will be picked up at the box office or other agreed upon location. In most cases this form will be required if you are ordering the same day the event is taking place or if FedEx can not deliver by the event date. For example you order on Friday night and the event is on Saturday. Since FedEx can not pick up the tickets we will have to leave them at the box office for you. Do note leave to pick up your tickets until you receive our confirmation with complete pick up instructions. |

A note about credit
card fraud:
Our company has zero tolerance for credit card fraud. We report all fraudulent
activities to the proper authorities. We have successfully helped prosecutors
arrest and convict those who have attempted to defraud us. Credit card fraud
is a federal crime and we will pursue fraudulent users both criminally and
civilly. Do not order if you are not an authorized user of the card.
A note if you are under 18 years old:
Please have your parent or guardian order for you. Do not use your parents
